Following the death of his wife, the world of Karl Ruisson begins to collapse little by little. After he leaves his family, having punched his older brother Gary, who resents Karl for being adopted and often badmouths Anna, his deceased wife, he starts walking the 12-mile path to his home in Vancouver, WA. In this moment, he is approached by three strangers, who state they need a dollar for the boatman so that they can cross the river. Confused, Karl tells them he doesn't have any money.

As he tries to get away, they attack him, leaving him to pass out on the street. He is then woken up by Vince, a tall black man who insists they have to go, and helps Karl to his feet after somehow healing his injuries. Karl starts to believe he probably drank too much and is simply having a crazy, alcohol-induced dream, and so decides to follow Vince, who welcomes him to the first step before hell, Limbo.

Unbeknownst to him, however, the fight for his soul has escalated, and this very journey home sends him careening through nightmares worse than any he could imagine. 12 miles lie between Karl and his salvation, or something far worse than damnation.
